IBCLC LACTATION CONSULTATION  |  SERVICE AGREEMENT

Compassionate, evidence-based infant feeding support

Practitioner: Sabira Camp, IBCLC  •  www.breathemamadoula.co.uk

Thank you for choosing Breathe Mama Doula. My aim is to offer thoughtful, respectful and individualised support. Please read this agreement before signing; it explains what is included and the terms of your booking.

1  |  Scope of practice

As an International Board Certified Lactation Consultant (IBCLC), I provide specialist, evidence-based infant feeding assessment, education and support. Depending on your needs, our consultation may include:

·        Discussion of your feeding journey, concerns and goals; observation of feeding, positioning and attachment.

·        Support with breastfeeding, expressing, bottle feeding, combination feeding and milk supply.

·        Assessment of feeding-related concerns, including oral function where appropriate; practical guidance and an individualised plan.

·        Signposting or referral to other healthcare professionals where appropriate.

I do not prescribe medication, perform tongue-tie division or provide medical treatment outside my professional qualifications. My service does not replace care from your GP, midwife, health visitor or paediatrician.

2  |  Fee, booking and payment

The initial home consultation costs £180 for up to two hours. Full payment is required in advance to secure the appointment; your booking is confirmed when payment has been received. Any additional services will be discussed and agreed separately.

3  |  Follow-up support

The £180 fee covers the agreed home consultation only. Further appointments, telephone consultations and ongoing advice by WhatsApp or other messaging are not included and are charged separately. Fees will be explained and agreed before additional support is provided.

4  |  Cancellation and rescheduling

I appreciate that circumstances with a new baby can change. Please give at least 24 hours’ notice if you need to cancel or rearrange.

·        With at least 24 hours’ notice, I will offer rescheduling without a rescheduling fee, subject to availability. If you cancel, refunds remain subject to your statutory rights and any lawful, reasonable costs already incurred.

·        With less than 24 hours’ notice, a fair and proportionate cancellation charge may apply only where permitted by law, reflecting losses or costs that cannot reasonably be avoided. Any remaining balance will be refunded.

·        If I cancel the appointment, you may choose an alternative date or receive a full refund for the service not provided.

5  |  Consent and professional boundaries

Your comfort, dignity and choices are central to my practice. I will explain any feeding observation, physical assessment or hands-on support and ask your permission first. You may decline or stop any element at any time. Recommendations are based on the information available during our consultation; specific feeding outcomes cannot be guaranteed.

6  |  Confidentiality and privacy

I keep consultation notes securely using Cliniko, a practice-management system for healthcare professionals. Cliniko uses encryption and access controls to help protect records. I am responsible for how your information is used, and Cliniko processes information on my behalf. The integrated Privacy Notice below explains this in more detail.

7  |  Privacy Notice — How I use your information

Legal bases: Health information receives additional protection under UK GDPR; I will rely on an appropriate special-category condition for providing healthcare under duties of confidentiality, or explicit consent where required.

Cliniko and security: I use Cliniko to store consultation records. I may share relevant information with other healthcare professionals when you authorise this or another lawful basis applies, and where required for safeguarding or by law. Necessary service providers, such as Cliniko and payment or signing providers, may process limited information on my behalf.

Storage and retention: records are kept only for as long as necessary for care, professional requirements, safeguarding, insurance and legal obligations

Your rights: subject to legal exceptions, you may request access, correction, erasure, restriction or transfer of your information, object to certain uses, or withdraw consent where processing relies on consent. Contact me using the details above.
